Out of Facility Form

Thank you for using Pawsitivity Rescue Inc. for your out of facility needs. This Form is for those looking to find a furever home for their companion when the rescue is full or unable to assist with your specific need.

To utilize this service please fill our our form or for those who prefer the pen and paper method, PDF download this form and fully fill out all information! Include a photo of your pet as well and  either scan and email both the completed form and photo to pawsitivityrescueinc@gmail.com  or mail it to 1420 N 2nd E. Mountain Home, ID 83647. Our team will review and post your pet for 60 days! After which time we will review and make any necessary changes.

Please note that all pets MUST BE spay/neutered before we will post them on our page for adoption as that is a key point in our mission to slow the cycle of animal overpopulation.
